See "Requirements for Membership" at the bottom of this Application before filling out the form. Your application will be reviewed by the Board of Directors and you will be notified how to proceed after the review is complete.

Fees:	New Member Administrative Fee: $50.00 - This one-time fee for inclusion on KWPA website
	Annual Membership Dues: $150 – Annual fee for listing under your primary category
	Additional Category Fee: $100 – Annual fee for each additional category, if needed

Requirements for Membership: In order to become a member of the KWPA you must:

1. Have been working on Kauai for at least one (1) year in the wedding or ancillary business.
2. Currently live and work on Kauai
3. Be sponsored by three (3) current KWPA members in good standing (see below)
4. Fill out this form in its entirety

If your membership application is accepted, we will email this form back to you. Please print it and send with your check for $200 (or more if you have Additional Categories) to: KWPA Membership, PO Box 203, Lihue, HI 96766. You will receive a New Member Packet, including information on providing material for your KWPA website listing (three (3) photos and text).
